Oak Meadow School students have requirements and options to attend various classes and other school events and activities. All Oak Meadow students see Class Teachers on a regular basis. While Oak Meadow’s independent study program is designed to be as safe as possible, there are inevitable risks associated with participation in the school.
Oak Meadow School does not carry medical insurance for injuries suffered by students while involved in its programs, classes, activities, or events.
Enrollment in Oak Meadow School requires that parents sign two documents which specifically transfer all risk to the families of enrolled students: this Disclosure of Risk, and a Waiver and Assumption of Risk.
Parents enroll their students in Oak Meadow School at their own risk. Those risks include, but are not limited to:
____ 1. Automobile accidents traveling to and from meetings with Class Teachers, classes, activities, or other school sponsored events.
____ 2. Injury suffered during participation in meetings with Class Teachers, or in classes, activities or events sponsored by the school. These may include slips and falls, particularly on the stairs at the church facility used for many of our classes and activities; the danger of accidental injury as students play with each other (chasing, roughhousing, throwing objects, etc.); the danger of students deliberately inflicting injury on each other in roughhousing or fight; the danger of students acquiring dangerous drugs or other substances from other students; the danger of students being struck by automobiles either in the parking lot at the church facility, around the home(s) of Class Teachers before, during or after meetings there; the danger of students leaving the church facility “campus” either with or without permission and being injured by automobile accident, being struck as a pedestrian crossing city streets, being accosted in the places of business in the neighborhood of the church facility; or other unforeseeable causes of danger.
____ 3. The danger of being involved in classes, activities, events, and programs in which students are NOT under immediate and constant observation and supervision. Oak Meadow’s independent study program by its nature sets out to foster independence on the part of its students, and that means that there will be risks involved.

signature below, affirm that I/we have read this Disclosure of Risk
carefully. I understand that no one can humanly predict in advance
every possibility for danger and mishap. By my signature on the
reverse, I/we voluntarily accept and assume these risks and any
others not discussed and predicted above, of my/our own free
will.
