Waiver and Assumption of Risk

I/we, __________________________________________________, the parent(s) of

_____________________________________________________, student(s) enrolled at Oak Meadow School, voluntarily sign this Waiver and Assumption of Risk in favor of Oak Meadow School, its directors, officers, administrators, independent contractor teachers, and adult volunteer supervisors or helpers, in consideration for the opportunity of enrollment in Oak Meadow School, participation in any and all classes, activities, events or gatherings offered or sponsored by Oak Meadow School, and contact with its various teachers.

I/we understand that there are certain risks and dangers associated with the above activities and the use of the various facilities where the classes, activities, events or gatherings are held, and that these risks have been fully explained to me. I/we fully understand the danger involved.

I/we fully assume the risks involved as acceptable to me/us, and I/we agree to use my/our best judgment in undertaking these activities, and to follow all safety instructions.

I waive and release Oak Meadow School, its directors, officers, administrators, independent contractor teachers and adult volunteer supervisors or helpers, from any claim for personal injury, property damage, or death that may arise from the enrollment of my child in, or his/her/my/our participation in the activities or instruction offered by Oak Meadow School.

I/we am/are a competent adult, and I/we assume these risks of my/our own free will.
